It can affect menstrual cycles, ovulation, skin and hair, fertility, body weight and metabolic health.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Importantly, <strong>PCOS is not simply an ovarian problem.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It is a complex hormonal and metabolic condition that can affect a woman&#8217;s health in different ways throughout her life.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The good news is that PCOS can be managed.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The aim is not simply to \u201cmake the periods regular.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Good PCOS care looks at the <strong>whole woman<\/strong>\u2014her symptoms, metabolic health, fertility goals, emotional wellbeing and long-term health.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">What Is PCOS?<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">PCOS stands for <strong>Polycystic Ovary Syndrome<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It is a condition involving hormonal imbalance and abnormal ovulation.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Women with PCOS may have higher-than-usual androgen activity. Androgens are hormones often referred to as \u201cmale hormones,\u201d although women naturally produce them too.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Higher androgen activity can contribute to:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Excess facial or body hair<\/li>\n\n\n\n<li>Acne<\/li>\n\n\n\n<li>Oily skin<\/li>\n\n\n\n<li>Hair thinning or female-pattern hair loss<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">At the same time, ovulation may become irregular, resulting in:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Irregular periods<\/li>\n\n\n\n<li>Infrequent periods<\/li>\n\n\n\n<li>Missed periods<\/li>\n\n\n\n<li>Difficulty becoming pregnant<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">PCOS can also be associated with insulin resistance and increased metabolic risk.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Is PCOS Really About Cysts?<\/h3>\n\n\n\n<h3 class=\"wp-block-heading\">This is one of the biggest misunderstandings about PCOS.<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Despite its name, <strong>you do not need to have ovarian cysts to have PCOS.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Some women with PCOS have ovaries that appear polycystic on ultrasound.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Others do not.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The \u201ccysts\u201d seen on an ultrasound are usually numerous small follicles\u2014not necessarily dangerous ovarian cysts.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">So:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>No cysts on ultrasound does not automatically rule out PCOS.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Cysts on an ultrasound do not automatically mean you have PCOS.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Diagnosis requires looking at the overall clinical picture.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">What Causes PCOS?<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The exact cause of PCOS is not completely understood.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It appears to involve a combination of:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Genetic factors<\/li>\n\n\n\n<li>Hormonal factors<\/li>\n\n\n\n<li>Insulin resistance<\/li>\n\n\n\n<li>Environmental and lifestyle influences<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">PCOS can run in families, and having a family history of PCOS or type 2 diabetes can increase risk.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This is why blaming a woman for developing PCOS is completely inappropriate.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u092f\u0939 \u0939\u0948 <strong>not simply because she ate too much sugar<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It is also not simply because she is overweight.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In fact, PCOS can occur in women of different body sizes.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">PCOS and Insulin Resistance<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">This is particularly important.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Many women with PCOS have some degree of insulin resistance.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Insulin is a hormone that helps glucose move from the blood into the body&#8217;s cells.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">When the body becomes less sensitive to insulin, the pancreas may produce more insulin to compensate.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Higher insulin levels can contribute to hormonal changes that make PCOS symptoms worse in some women.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This can create a difficult cycle:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Insulin resistance \u2192 hormonal changes \u2192 easier weight gain \u2192 further metabolic problems<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But this does not mean every woman with PCOS has insulin resistance, nor does it mean that weight loss is the only treatment.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The 2023 international guideline specifically recognizes insulin resistance as an important part of PCOS physiology, while noting that routine clinical insulin assays are not very useful for diagnosing insulin resistance.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Common Symptoms of PCOS<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">PCOS can look very different from one woman to another.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">One woman may mainly have irregular periods.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Another may struggle with acne and facial hair.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Another may have difficulty becoming pregnant.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Another may have few obvious symptoms but still have metabolic risk.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Common features include:<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">1. Irregular Periods<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">Periods may:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Come very late<\/li>\n\n\n\n<li>Occur only occasionally<\/li>\n\n\n\n<li>Be unpredictable<\/li>\n\n\n\n<li>Sometimes be absent<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Irregular ovulation is one of the major features of PCOS.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">2. Excess Facial or Body Hair<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">Higher androgen activity can cause increased hair growth on areas such as:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Upper lip<\/li>\n\n\n\n<li>Chin<\/li>\n\n\n\n<li>Jawline<\/li>\n\n\n\n<li>Chest<\/li>\n\n\n\n<li>Abdomen<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">This is called <strong>hirsutism<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It can be particularly distressing emotionally and socially.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">3. Acne and Oily Skin<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">Adult acne that continues beyond the teenage years can sometimes be associated with androgen excess.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But acne alone does <strong>\u0928\u0939\u0940\u0902<\/strong> mean that you have PCOS.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">4. Hair Thinning<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">Some women experience thinning of the hair on the scalp or a female-pattern distribution of hair loss.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Again, this symptom can have many causes, so it should not automatically be attributed to PCOS.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">5. Difficulty Becoming Pregnant<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">Because ovulation may be irregular, some women with PCOS have difficulty conceiving.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">PCOS is one of the leading causes of infertility related to failure to ovulate.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But there is an important message here:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>PCOS does not mean you cannot become pregnant.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Many women with PCOS become pregnant with appropriate lifestyle support, ovulation treatment or fertility treatment when needed.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">Can You Have PCOS Without Being Overweight?<\/h4>\n\n\n\n<h4 class=\"wp-block-heading\">Absolutely.<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">This is another important myth.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">PCOS is not a condition that affects only women with obesity.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A woman can have a normal body weight and still have:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Irregular periods<\/li>\n\n\n\n<li>High androgen levels<\/li>\n\n\n\n<li>Acne<\/li>\n\n\n\n<li>Excess facial hair<\/li>\n\n\n\n<li>Ovulation problems<\/li>\n\n\n\n<li>Insulin resistance<\/li>\n\n\n\n<li>Metabolic risk<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">This is sometimes referred to informally as <strong>\u201clean PCOS,\u201d<\/strong> although it is not a separate type of PCOS.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The correct approach is to assess the individual woman rather than assuming her diagnosis from her body size.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The international guideline emphasizes healthy lifestyle and prevention of excess weight gain even in women who are not overweight.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">PCOS and Weight Gain<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">For some women, weight management can be particularly difficult with PCOS.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Hormonal and metabolic changes may contribute to weight gain or make weight loss harder.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But we should be careful about one common message:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>\u201cIf you have PCOS, just lose weight.\u201d<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That is not good medical advice.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A woman can have PCOS at any body weight.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And healthy lifestyle is valuable <strong>even when it does not produce weight loss<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The international guideline recommends healthy lifestyle intervention for all women with PCOS and emphasizes prevention of excess weight gain, metabolic health and emotional wellbeing.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">How Is PCOS Diagnosed?<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">There is no single blood test that says:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>\u201cYes, you have PCOS.\u201d<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Diagnosis involves looking at symptoms, medical history, examination and appropriate tests while excluding other conditions that can cause similar symptoms.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For adults, PCOS is generally diagnosed when <strong>at least two of three features<\/strong> are present after other causes have been excluded:<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">1. Irregular or absent ovulation<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">Usually reflected by irregular menstrual cycles.<\/p>\n\n\n\n<h4 class=\"wp-block-heading\">2. Evidence of androgen excess<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">This may be:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Clinical signs such as excess facial\/body hair or acne<\/li>\n\n\n\n<li>Or elevated androgen levels on blood testing<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">3. Polycystic ovarian morphology<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">This can be identified using ultrasound, and in adults, the 2023 guideline also recognizes <strong>AMH as an alternative to ultrasound in appropriate diagnostic settings<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Not every woman needs every test.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The correct diagnostic approach depends on age, symptoms and clinical circumstances.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Why Other Conditions Must Be Excluded<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Irregular periods and excess hair can have many causes.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For example, doctors may need to consider conditions involving:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Thyroid function<\/li>\n\n\n\n<li>Prolactin<\/li>\n\n\n\n<li>Adrenal hormones<\/li>\n\n\n\n<li>Pregnancy<\/li>\n\n\n\n<li>Other hormonal disorders<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">That is why diagnosing yourself based on a social-media checklist is not a good idea.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">PCOS should be diagnosed by looking at the complete clinical picture.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">What Tests May Be Needed?<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Depending on your symptoms, your healthcare professional may consider:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Pregnancy testing when appropriate<\/li>\n\n\n\n<li>Hormone testing<\/li>\n\n\n\n<li>Thyroid testing<\/li>\n\n\n\n<li>Prolactin<\/li>\n\n\n\n<li>Androgen measurements<\/li>\n\n\n\n<li>Blood glucose assessment<\/li>\n\n\n\n<li>Lipid profile<\/li>\n\n\n\n<li>Blood pressure measurement<\/li>\n\n\n\n<li>Ultrasound in selected cases<\/li>\n\n\n\n<li>AMH in selected adult diagnostic situations<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The tests should be chosen based on your symptoms and medical history.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">More tests do not always mean better diagnosis.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>The right tests are better than unnecessary tests.<\/strong><\/p>\n\n\n\n<h3 class=\"wp-block-heading\">PCOS Is Also a Metabolic Condition<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">This is an area that deserves much more attention.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">PCOS is associated with an increased risk of:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Insulin class=\"wp-block-paragraph\">This is why PCOS management should not stop once the periods become regular.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Your long-term metabolic health matters too.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">PCOS and Diabetes Risk<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Women with PCOS have an increased risk of abnormal glucose metabolism and type 2 diabetes.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This is particularly relevant when PCOS occurs together with:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Excess wait for symptoms of diabetes.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Ask your healthcare professional whether you need appropriate glucose screening.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">PCOS and Mental Health<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">PCOS can affect much more than hormones and periods.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Women may struggle with:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Anxiety<\/li>\n\n\n\n<li>Depression<\/li>\n\n\n\n<li>Poor body image<\/li>\n\n\n\n<li>Stress related to infertility<\/li>\n\n\n\n<li>Distress about facial hair or acne<\/li>\n\n\n\n<li>Weight-related stigma<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The international guideline emphasizes psychological wellbeing and quality of life as important parts of PCOS care.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This matters because a woman may look perfectly healthy from the outside while struggling significantly with the emotional effects of PCOS.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>PCOS care should treat the person\u2014not just the laboratory results.<\/strong><\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Can PCOS Be Cured?<\/h3>\n\n\n\n<h4 class=\"wp-block-heading\">There is currently no permanent cure for PCOS.<\/h4>\n\n\n\n<p class=\"wp-block-paragraph\">But that does <strong>\u0928\u0939\u0940\u0902<\/strong> mean you have to suffer with symptoms forever.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">PCOS can be hair<\/li>\n\n\n\n<li>Ovulation<\/li>\n\n\n\n<li>Fertility<\/li>\n\n\n\n<li>Metabolic health<\/li>\n\n\n\n<li>Quality of life<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The right treatment different for each woman.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">1.
